2. Choose the form for the date
Forms differ for crimes before January 1, 2024 and crimes on or after that date.
If a crime occurred over a period of time, the Government of BC directs applicants to use the form corresponding to the date of the last incident. Use the official forms page so you receive the current version.
3. Gather what the official form asks for
- Applicant and contact information
- Information about the incident
- Police or other report information, if available
- Information about injuries, treatment, expenses and other available coverage
- Supporting documents requested for the benefits you are seeking
4. Complete the application
The Government of BC offers an online application and downloadable PDF forms. The online form says it takes about 45 minutes and cannot be saved for later. For a PDF digital signature, follow the current instructions carefully; typed signatures are not accepted by the program.
